I'm An Artist Community Studio Participant Waiver and Parent Consent Agreement

(Please check 'yes' via the registration process if you agree to these terms & please retain a copy for your records)

 

The following guidelines and expectations are in place for children and families to make the most of their time at I'm An Artist Community Studio School.

     Attendance:

No mandatory attendance will be taken. However, when you register certain guidelines should be agreed upon and followed for the safety and consideration of your student, the class, and for myself.

      Check-In Policy:

  • Each child will be checked in for each class by their guardian or designated person. 

  • Guardians are responsible for reminding their children of class times as well as taking care of transportation and arrival in a timely manner

  • Guardians, you are responsible for notifying me if your student will miss a class ASAP. This allows me to focus my attention on teaching the class and ensures your child’s safety. Repeated failure to communicate your child’s absences may result in your child being dropped from the class for the remainder of the 6-weeks.

  • In the event this occurs, no refunds will be given.

      Pick up policy:

  • Please be aware of what time class ends and make sure that you or your designated person are on time to pick up your child. All children must be signed out within 5 minutes of the end of class in order for me to have time to prepare for the next class.

  • If you are going to be late, please notify me immediately.

  •  Parents or the designated person are responsible for signing students out and transportation from class.   Transportation is not provided.

  • Children will be released only to adults.

In the event that your student will be using public transportation, please contact me regarding early dismissal if needed.  (Bus 6-Eastown/Woodland runs in front of the studio)

     Code of conduct:
Students are expected to maintain the same behavioral standard that they would have in the world operating as young adults.  Please understand that while this is an art class which is fun in nature, it is also a time for growth, observance, persistence, learning, creating and pushing boundaries.  Respect for self and others is a must.  I'm An Artist is a safe space and has a zero-tolerance policy against harassment and bullying of any sort.  Please talk with your child about the expectations before classes start.  If disruptive behavior becomes an issue during class time, guardians will be contacted in order to make a behavior plan. However, if the pattern continues, students may be removed from the class for the session. No refunds will be provided. Parents will be given a written notice before these measures are taken. However if the pattern persists, it will be discussed with the guardian and after two times the child will be removed from class.
     Snacks:
Nut-free, healthy snacks will be provided throught the day.
     Allergies/medical conditions:
Guardians of students with severe allergies and/or medical conditions are responsible for contacting myself in order to develop a formal health plan. I do not have access to medical supplies in the studio.  Please send any epi-pens or other medication that might be needed with your student to each class and contact me directly about usage.

     Educational Considerations:

If your child has any social, personal or educational differences you think I should know about, and some ways you've found to be helpful in their education please contact me to discuss a plan, if needed.  This information will not be shared and is only to best serve your child.
     Emergency treatment:
By agreeing to this form, guardians give permission for their children to participate in I'm An Artist Community Studios Enrichment Classes and give permission to the adult in charge to seek emergency medical treatment and/or transportation for their children. 
     Backup Childcare Plans:

Please note that we will be follow Kent County in the calling of snow days. If Kent is listed on the news, please don't make the drive.  If there is a two-hour delay for East Grand Rapids Schools, we will not have our morning class for that day.

  • Please have a backup plan in place in case this occurs.

If I have a personal unforeseen emergency, I will contact everyone ASAP. 

  • Please have a backup plan in place in case this occurs.

Please ensure your child’s safety and well being by having a backup plan and communicating that plan to your child and to the emergency contacts listed on your child’s registration form.

      Adult/Student Ratio:

  • For safety and education purposes, class ratios are kept to a minimum.  There will be times where two instructors are available in which case class sizes may increase. 

 

Open Studio Time

We are happy to offer open studio time of Tuesdays, Wednesdays and Thursdays from 3-5.  This is a great way to further creative time, try new mediums and mingle.

  • Please let me know if your child plans to attend and you will not be picking them up at the end of class.

  • Please understand that during this time, your child is no longer in the official care of I'm An Artist Community Studio. This is a time that people come and go as they want. 

  • Please have a conversation with your child about this, and have a pick-up plan in place.

  • Thank you for understanding

CANCELLATION POLICY

We understand that life happens and sometimes we need to make adjustments.  At I'm An Artist you may cancel your class with a refund up to 3 weeks before it begins, however after point a credit toward another class within a year can be issued.  There are no refunds after the 3 week point.

 

LIABILITY:
IN CONSIDERATION of being permitted to participate in any classes at I'm An Artist Community Studio, I, for myself, my child (and any personal representatives, assigns, heirs, and next of kin of myself or my child), acknowledge, agree and represent that:
❏ I UNDERSTAND (and in the future will ensure that I understand) the nature of the activities in which I register my child to participate, and I fully ASSUME all risk of, and responsibility for, determining my child’s eligibility and fitness to participate in such activities. I also will notify I'm An Artist Community Studio if the above information
changes.
❏ I HEREBY RELEASE, DISCHARGE, AND AGREE NOT TO SUE the I'm An Artist Community Studio or its owner, employees, volunteers, or other participants (each a “Release” and collectively the “Release's”) from ANY AND ALL LIABILITIES, CLAIMS, DEMANDS, LOSSES, OR DAMAGES caused, or alleged to be caused, in whole or in part, by my child’s participation in activities or by the omissions or acts of any Release's (including negligent rescue operations), to the fullest extent permitted by law.
❏ I FURTHER AGREE that if, despite this agreement, I, or anyone acting on my or my child’s behalf, makes a claim against any of the Releases in connection with the activities, I will INDEMNIFY, SAVE, AND HOLD HARMLESS each of the Release's from any liabilities, damages, losses or expenses (including attorneys’ fees and court costs), which they may incur as the result of such claim.
I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND AGREE FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE, INTENDING IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. I FURTHER AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID, THE BALANCE WILL CONTINUE IN FULL FORCE AND EFFECT.
By checking the 'yes' box during the registration process, you legally agree to the above agreements in lieu of a paper signature.
